Guillana Simon, FNP
Guillana “Lenny” Simon, MSN, FNP-BC, is a Family Nurse Practitioner with more than two decades of experience caring for patients across emergency medicine, urgent care, and geriatric primary care settings.
Lenny began her nursing career in the Philippines, where a strong cultural respect for elders and community-centered care shaped her lifelong commitment to serving older adults. After moving to the United States, Lenny worked for more than two decades in high-acuity emergency departments and urgent care centers across Maryland, Virginia, and Washington, DC, which include hospital-based emergency rooms, ambulatory care settings, telehealth, and home-based settings. Through this work, Lenny repeatedly saw older adults cycle through hospitals simply because accessing consistent primary care became difficult.
After years of seeing older adults struggle to access consistent medical care, Lenny founded Provita Home Health to bring thoughtful, reliable primary care directly to patients’ homes. Her work is grounded in dignity, cultural humility, and a commitment to caring for communities that are too often overlooked.

What We Offer at Provita
Provita Home Health provides home-based medical services for seniors in Montgomery County and some parts of Prince George’s, Howard County, and Southern Maryland. All services are delivered through in-home visits and telehealth, with a focus on prevention, early intervention, urgent care, and long-term continuity of care. The provider can order diagnostics such as blood work, X-rays, EKGs, ultrasounds, and 2D-echoes that can be done from the comfort of your home. The provider can refer to specialists and to in-home physical, occupational, and speech therapy. Care is designed to reduce unnecessary hospital visits, support aging in place, and provide a reliable medical partnership for patients and caregivers alike.

Primary Care and Preventative Care
Ongoing primary care delivered in your home, focused on prevention, routine medical needs, lab review, imagings and coordination with specialists. Designed for continuity, accessibility, and long-term health support for homebound seniors.

Geriatric and Aging Care
Specialized care for older adults focused on independence, safety, and quality of life. Services include cognitive screening, medication review, preventive aging care, and support for caregivers and family members.

Same Day and Sick Visits
Evaluation and treatment of new or worsening symptoms without the stress of urgent care or the emergency room. Same-day sick visits address common illnesses, sudden changes, and acute concerns directly in the home.

Chronic Disease Management
Long-term management of chronic conditions such as hypertension, diabetes, asthma, COPD, and cardiovascular risk. Care includes medication management, monitoring, lab review, and coordination with specialists as needed.
