Meet Guillana
My name is Guillana “Lenny” Simon, MSN, FNP-BC, and I am a Family Nurse Practitioner with a deep commitment to caring for older adults who are often overlooked by traditional healthcare systems. I began my nursing career in the Philippines, where a strong cultural respect for elders and family-centered care shaped how I view medicine—not just as treatment, but as service. That foundation has guided my work throughout my career.
After moving to the United States, I spent many years working in emergency departments, urgent care centers, post-anesthesia care units, and infectious disease clinics across Maryland, Virginia, and Washington, DC. While these high-acuity settings strengthened my clinical judgment and decision-making, they also revealed a troubling pattern: many older adults were ending up in the hospital simply because accessing routine, consistent care had become too difficult.
That realization led me to pursue advanced training at The George Washington University and ultimately to found Provita Home Health. I created Provita Home Health to bring primary care into the home, where I can better understand patients’ daily lives, medication challenges, safety concerns, and support systems. This approach allows for care that is practical, respectful, and truly patient-centered.
Today, I focus on home-based primary care and geriatric medicine, working closely with patients and families to support aging with dignity, continuity, and compassion.
